Series Waivers

To participate in any Formula Pro USA events, all drivers, team members, and participants must have a completed FPUSA Annual Waiver on file. Formula Pro USA utilizes Adobe Sign for a secure, paperless process. Please follow the instructions below to ensure your documentation is processed correctly.

An annual waiver is required for:

• All Drivers (F3, F4, and WRS classes)
• Crew Members & Team Staff
• Family & Guests (Anyone requiring access to restricted areas/hot pits)

The adult waiver is a simple 3-step process:

1. Access the Form: Click the “Complete FPUSA Adult Waiver” button below.
2. Sign & Date: Follow the highlighted prompts in Adobe Sign to enter your name, birthdate, signature date, and electronic signature.
3. Confirm: Once you click “Submit,” you will receive a final PDF copy via email for your records. The FPUSA Series office will automatically receive your completed waiver.

Drivers under 18 years of age require signatures from both themselves and a Parent/Legal Guardian:

1. Initiate as Parent/Guardian: Click the link below. The Parent or Legal Guardian should be the one to begin the process.
2. Parent/Guardian Signature: Fill out the “ANNUAL PARENT/LEGAL GUARDIAN OF MINOR CONSENT, RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT” portion of the waiver package first and digitally sign that section.
3. Parent/Guardian: Next fill out the “ANNUAL RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT MINOR (<18)” portion of the waiver package (with the exception of the MINOR Legal Signature) and digitally sign that section as well.
4. Parent/Guardian: Enter your email address and click on “Click to Sign” at the bottom of the page.
5. Parent/Guardian: A box will open titled “Assign to next participants” prompting the entry of Participant 2 information. Fill in the MINOR’s First Name, Last Name and Email Address and click on “Click to Sign”.
6. Parent/Guardian Verification Email (Crucial): You will receive an email from Adobe prompting you to “Confirm my email address” click on this to confirm.
7. Minor Driver: Adobe Sign will send a notification to the Second Participant (MINOR) for their signature.
8. Minor Driver Signature: Ensure the Minor driver signs the “Minor Waiver” section as indicated by the signature fields.
9. Minor Driver: Click on “Click to Sign” at the bottom of the page.
10. Completion: Both the Parent/Guardian and the FPUSA Series office will receive a copy of the fully executed document once finalized.

• Annual vs. Event Waivers: The FPUSA Annual Waiver Package is valid for the entire Annual Season. You only need to complete this digital package once per year.
• Age Restrictions: Drivers must normally be a minimum of 14 to 16 years old (depending on the host track and sanctioning body) upon the first day of the event. Drivers younger than 14 may be evaluated on a case by case basis. The ultimate eligibility of a Driver to race in the series is subject to the host track and sanctioning body for that race event.
• Additional Waivers: In addition to the FPUSA Series Waiver, drivers and crew may be required to sign separate sanctioning host (e.g., SCCA, NASA, SVRA, etc.) or track-specific waivers at the gate for each event.
• No Paper Needed: If you complete the Adobe Sign process successfully, you do not need to print or email a copy of the FPUSA Annual Waiver to the FPUSA Series office; it is tracked automatically in our system.
